Financial Options

At Maple Family Dentistry, we are proud to provide our patients with the best dental care that our patients deserve.  It is our desire to help our patients overcome any financial barriers that they may encounter in achieving excellent oral health.  To do this, it is important that we do not allow dental insurance benefits to be a determining factor in our diagnosis. Your treatment will be based upon your needs and we assume that you are as concerned as we are about maintaining your optimal dental health.

Our fees are based on the care, skill, judgment and time needed to provide appropriate treatment.  In general, our office follows the fee guide as set out by the Ontario Dental Association.  We believe in open communication, and will carefully discuss treatment and associated fees with our patients prior to beginning any treatment.

Payment for services is due at the time services are rendered. Full payment is expected on your initial visit unless pre-appointment arrangements have been made with your insurance.  To assist our patients with their healthcare investment, we provide the following payment options:  cash, debit, credit cards (Visa, Mastercard, American Express).  Individual specific payment plans may be created in special circumstances and complex treatment cases.  3-month interest-free payment plans will be accepted upon patient providing a 50% deposit prior to commencement of major treatment. Invisalign treatment balance (following initial downpayment for laboratory fees) may be amortized over the duration of treatment.

Fees estimates listed for dental treatment can only be extended for a period of three months from the date given. Balances older than 90 days will be directed to a collections agency and may be subject to additional collection fees and interest charges of 1.5% per month.

We will gladly discuss your proposed treatment and answer any questions related to your insurance. However, you must realize that:

1.       Your insurance is a contract between you, your employer and the insurance company. We are not a party to your contract.

2.       Not all services are covered benefits in all contracts. Some insurance companies arbitrarily select certain services they will not cover.  If you have further concerns regarding this, please contact your company human resources department.

3.       Insurance benefit assignments will be paid to our office. The co-payment will be collected from you at the time of services rendered.

Our office understands the value of insurance benefits to our patients. If you have dental insurance, we ask you to bring the necessary information from your insurance company.  We do accept assignment from insurance plans, and will be happy to assist patients in filing their claim.  However, the balance remaining (co-payment) from any dental treatment will be collected on the same day as services are rendered.  Although we cannot always guarantee insurance coverage payments or benefits, we can submit an insurance pre-determination on our patient's behalf prior to beginning any " major" dental work.  Because of the complexities of dental insurance requirements, we provide this assistance for you as a courtesy.  However, the primary responsibility for dealing with your company is fundamentally yours, not ours.  Disagreements and misunderstandings are not between this office and the insurance company, but rather between you and the insurance company.  Your personal involvement will help to insure the best possible response and service.  Your insurance company is far more likely to respond to requests or complaints directly from you rather than from us.
